I understand that Your Tempo Healing is not paneled with any insurance company and I cannot use in-network, medicare, or medicaid benefits to pay for services. I understand that should I elect to begin treatment with Your Tempo Healing that this means I am agreeing to pay all costs up front and will not be able to use my in-network benefits. I understand that should I elect to use my out-of-network (OON) benefits, that Your Tempo Healing is happy to provide a superbill so that I can submit to my insurance for reimbursement. Additionally, I am aware that Your Tempo Healing can help me file these claims with the support of a secondary service called Mentaya.

Please also note that, since Your Tempo Healing is an out of network practice, we are unable to work with anyone who currently uses medicaid or medicare for health insurance.

I understand that the initial intake costs $245 and recurring 55-60 minute appointments cost $235. I also understand that, should I need financial assistance, I can ask to learn more about Your Tempo Healing's sliding scale program.
